In 2015, the Hamilton Family Health Team embarked upon a consultation process that involved over 1,000 people across the greater Hamilton area. Our goal was simple: to listen to HFIT patients, our partners and team members, then come up with a new set of guiding principles to carry us into the future. Through careful consultation, listening, and a whole lot of note-taking.
We've come up with 10 new core objectives to take us through 2016 and beyond. The Hamilton Family Health Team supports a comprehensive service delivery model. We believe in a physician-led, team-based model of comprehensive care that integrates best evidence and continuous quality improvement in patient care. We strive for a high-quality provider and staff experience.
As an organization, we value and respect the health and well-being of our providers and staff, and support continuous learning, collaboration, and engagement. We pride ourselves on consistent access to services. We will always work to ensure our patients have access to care in a timely and effective manner. We believe in actively engaged patients.
Using educational opportunities and engagement strategies, the Hamilton Family Health Team encourages patients to be informed and responsible participants in their own care. We provide enhanced care coordination. Family practices play a leading role in care coordination. Our healthcare professionals facilitate and connect patients with hospitals, specialists, and other agencies to ensure patients receive the best care possible.
We promote decentralized, integrated services. Our interprofessional health providers and consultant physician services are predominantly decentralized and integrate within family practice settings and local hubs. Having a team of professionals where patients can best access them ensures that they will receive the right care at the right time with the right provider.
We will work to enable information technology. The HFIT supports the use of technology that connects providers, healthcare partners, and patients to enable high-quality clinical care, healthcare planning, and evaluation. We are focused on population health. We will always work to promote a population health focus that allocates health services and HFIT resources equitably.
We provide effective leadership and collaboration. The Hamilton Family Health Team leads primary care in creating effective linkages with health systems, hospitals, and community partners to improve access, coordination, and planning of health, and social services. And we will always advocate for health and wellness. We will strive for system improvement that optimizes health and well-being of the patients, families, communities, and populations we serve.
In short, we want to thank you for your engagement. Your thoughts. And comments during our strategic planning process. Now it's time to take what we've learned forward. forward and lead our teams, our partners, and our patients into this new era of HFID. We're ready to implement these core directions to ensure that our family health team provides the type of patient care and service we can all be proud of.

About this project
I have worked with Double Barrel on several projects with various organizations I am involved with. I can’t say enough about what a great team they are and the innovative work they do. They really get to know you and your project to better understand the vision for whatever it is you are trying to convey through video.
